Those are colored cocoa butter transfer sheets that were used to transfer the design to the finished chocolates.
They basically screen print a design using colored cocoa butter to an acetate transfer sheet. You then cut them to size and place them on a freshly dipped chocolate before it sets. The acetate is then removed after the chocolate has cooled and been fully setup, leaving behind whatever cocoa butter design was printed.

I'm not aware of any machine that will do that. It does use a screen printer or hand painting of the cocoa butter. You can purchase sheets at quite a few places, or you can have chefrubber.com do custom sheets. The foodsafe qualities of the emulsers and screen make it difficult and cost prohibitive to do on your own. <edit> I should clarify, the food-safe nature does not inherently make it difficult, however, obtaining materials is quite difficult. You can buy the colored cocoa butters easily enough, but screen materials are next to impossible to find that are also food-safe. Remember, it's the same process as making a printed tshirt; however, you generally dont eat t-shirts, so dangerous chemicals arent an issue there. </edit>

You can get printers which print coloured cocoa butter as the "ink". From what I hear, the subsequent transfers are most effective on white chocolate only.
Or you can go to producers of transfer sheets who will take your logo etc and make a customised transfer sheet for you (usually there is a minimum order size)

I'm also interested in making and designing chocolate transfer sheets myself. I'm just wonder if anyone know how to use colored cacao butter and print it on an acetate transfer sheet? If you know the machine that can do that. Please provide me the producer name and link. Thank you

The problem with deko (and any other edible-inkjet based system) is that the inks simply don't transfer well to dark chocolate. They will look great on white, but completely washed out and colorless on dark chocolate. This is because inkjet inks are translucent, not opaque like colored cocoa butter. Some people have tried spraying the background with white edible ink or CCB, with mixed results. The only way to make your own CCB transfer sheets is through silkscreening. There is a home system available for screen printing called the YUDU, but their emulsions are not certified as food-safe.
